Case Presentation:

Dr Subhash Jangid, Director and Unit Head, Bone and Joint Institute at Fortis Memorial Research Institute, Gurugram, along with our design engineers planned the whole prosthesis. As the previously implanted hip prosthesis was lying loose in the pelvis, this caused pain and constant irritation of the muscles surrounding the implant. This resulted in loss of sleep in patient. She was also wheelchair bound and barely able to go to washroom even with the support of walker. During these virtual surgery planning sessions each and every screw location was planned to be customized, the trajectories and the type of screws were decided as per bone thickness and bone stock availability to enable maximum bone purchase, lattice incorporation in the implant was also planned so as to reduce the weight of the implant. Various simulation tools were applied to estimate available bone stock and probable defect size/dimensions after removal of existing implants virtually. This trabecular lattice incorporation, apart from reducing weight of the implant has got clinical significance as it will give optimum Osseo integration. 3D printed customized/Patient Specific Implant (PSI) by Incredible AM Pvt Ltd. gave them a flexibility of having pre-defined trajectories, fair idea about the surgery and no surprises on surgery table. VSP-Virtual Surgical Planning, gives surgeons the clarity and helps them reduce actual surgery time. The trial of pre surgical planning could be performed using the VSP.
